Social Support
I would like social support to participate in community social activities
Social support enables an older person or person with a disability to participate in meaningful and enjoyable activities which are appropriate to their needs, interests and abilities and which support and encourage social interaction and mental stimulation.
It is aimed at maximising a person's ability to continue living independently at home and assist in meeting the person's need for social contact.
Social Support involves assistance provided by a companion, either a paid worker or a volunteer, to enable an older person or person with a disability to participate in community life. It may be provided either within the home environment or in the community.
Catholic Healthcare offers a comprehensive and integrated range of support services for frail aged, and people with a disability, to assist them to be more independent in the community and enhance their quality of life. These services include support programs for younger onset dementia clients, counselling support for individuals and families, a telephone links service for clients at risk of social isolation, one on one social support for clients and a range of day centres for frail aged and frail aged clients from CALD backgrounds. Services are flexible and tailored to meet the person's needs.
Social Support often achieves a dual outcome of providing a support program to a client as well as providing carers a break from their usual caring relationship.
