Mercy Community Care
Mercy Community Care is a Catholic Healthcare service that provides extensive community care services in the lower and upper north shore areas of Sydney. Designed for frail older people and people with disabilities living at home, Mercy Community Care supports the independence, health and well-being of its clients. In addition, the Mercy Community Care offers a range of support and respite services to carers. A full list of the Mercy Community Care's services are described below.
Home and Community Care
Community Aged Care Packages
Planned and coordinated care services to assist older people with complex needs to remain in their own homes.
Extended Aged Care at Home
Comprehensive nursing and in-home care to assist frail older people with high level needs to remain at home.
Extended Aged Care at Home Dementia (EACH D)
Provides the same range of services as 'general purpose' EACH packages. However EACHD packages offer service approaches and strategies to meet the specific needs of care recipients with dementia. Living Conditions
Supports people living in poor living conditions. Designed for people with disabilities, mental illness, memory loss or confusion associated with dementia and people who live alone or with someone who, through their own ill health or disability, is unable to provide care.

Dementia-specific care
Dementia Day Service
A long day care program for people with moderate to severe dementia in a small program supported by staff and volunteers.
Dementia Advisory Service
Advice, counselling and training on dementia-specific services for people with dementia and their carers.
Monitoring Program
Support for people with confusion or dementia who live alone or live with someone who is unable to provide care support to the person with dementia.
Younger Onset Dementia
A flexible and individualised social program for people under 65 years of age who have been diagnosed with dementia.
Dementia Options
A case management service that assists people with memory loss or dementia and who have multiple care needs.
Transition care
ComPack (Community Case Management Package)
Case management of care for up to six weeks after discharge from hospital for people who need two or more community services.
Sub Acute Fast Track Elderly Care (SAFTE)
Care support for older people living at home and whose medical conditions are starting to worsen.
Transitional Aged Care Program (TRANSPAC)
Packages of care and therapy for older people being discharged from hospital.

Recruiting, training and support for volunteers who assist in a variety of areas within a residential and community setting.
Community Links
A telephone link service that connects a volunteer with an older person living in the community who may be at risk due to social isolation. Volunteers make daily contact with the older person.
Community Visitors Scheme
A linking service that connects a volunteer with an older person living in a residential aged care service.
