How do I apply for residential aged care?
You can secure a placement at one of Catholic Healthcare's residential aged care services by completing the three part application process. You will need an ACAT assessment and a financial assessment, and you will need to complete the application for the residential aged care service you would like to enter. These three steps are described below.
Step 1 - ACAT assessment
To secure a placement at one of Catholic Healthcare's residential aged care services, you will need to have a current assessment from the Aged Care Assessment Team (ACAT). There is no charge for an ACAT assessment.
You must be assessed by an ACAT representative so that we can provide you with the most appropriate care. The formal assessment will be conducted by an ACAT representative, who will discuss your needs with you.
A member of the ACAT team will visit you and ask a series of questions. This helps them make an informed assessment of your abilities and your needs.
You can contact your local doctor or hospital or call the Aged Care Information Line on 1800 500 853 to find the ACAT in your area.
Step 2 - Financial assessment
You may be eligible for government assistance to help pay for your accommodation. To determine if you are eligible for assistance, you will need a financial assessment. You can get more information about this by contacting Centrelink or the Department of Veterans' Affairs (DVA).

Step 3 - Application
Prior to entering a Catholic Healthcare residential aged care service, you will need to complete some application forms specific to the service you would like to live in. You will need to contact that service to request an application form.
